How to Obtain ADA Certification

To use Valley Transit II because of a disability, individuals must first be issued an ADA Certification Card.

Here are the steps you need to take in order to become certified to use Valley Transit II ADA Paratransit Service:

1.  Please mail completed forms to Valley Transit at the address below. You can also fax forms to 920-830-7599. Click here for “Request for Certification of ADA Eligibility” form. 

2. Valley Transit will be responsive to all Requests for ADA Certification.  However, the certification process may take up to 21-days after Valley Transit receives all necessary information.  A personal visit for assessment may be required by Valley Transit.
